HEALTH POLICIES AND PROCEDURES FOR 2008-2009 SCHOOL YEAR

Dear Parents and Guardians:
To simplify your preparations for the upcoming school year, I have outlined a number of important policies and procedures relating to the health office:

All new students and those entering and returning to Mixed Age Kindergarten, Second Grade, Fourth Grade, Seventh Grade and Tenth Grade are required to have a physical exam by a New York State licensed Health Care Provider, no more than 12 months prior to their entrance into school. The physical exam must be presented to the Nurse before school begins and NO LATER than five days after the first day of school. (Form #1.) Please submit the Emergency Consent Form (Form #2) at the same time. |

If you are requesting a religious or medical exemption from the immunization requirement, you must file “An Exemption to Immunization Form” (available in Health Office) within 5 days of the first day of school. |

A Sport Physical Exam (Form #3, enclosed for grades 7-8 and high school) is required annually for any student participating in the after school sports program. Physical exams must be completed within 12 months prior to the start date of the sport. To cover all three sports seasons, please be certain that the exam is performed after June 1, 2008. Additionally, a Health History Update Form (White Triplicate) must be completed and signed by a parent no more than 15 days prior to the start of EACH sport season. Please be aware that students will not be allowed to “try out” or participate without both the Sport Physical and a Health History Update Form on file. |

Any student taking prescription or over-the-counter medication during school hours or on school trips must have an “Authorization for the Administration of Medication” form on file. For High School students who need to carry their medication with them, parents should also submit the “Self-Medication Release Form”. In addition to parent approval, these forms must be completed by a licensed Health Care Provider, and are kept on file in the Health Office. Medications to be administered by the school Nurse must be delivered by an adult to the Health Office in the container dispensed by the pharmacy; or, if a non prescription, in the original container only. |

Any student who sustains an injury or has a medical illness which prevents or limits their participation in Physical Education/Sports should submit a note to the Nurse, from their Health Care Provider, stating the diagnosis and the student’s restrictions from participation. A follow up note from the Health Care Provider is REQUIRED when the student can return to full participation. |

Any student who has sustained an injury requiring the use of crutches, a cane or a wheelchair in school must present a medical note from a Health Care Provider stating the diagnosis and the need for the use of crutches, cane or wheelchair in school. |
