Maple Pointe Assisted Living Permission Slip

I give my child permission to participate in the Maple Pointe Assisted Living trip on Wednesday December 14, 2016. I understand that the trip will be departing at 10:30am via Waldorf Activity Vehicle, and returning at approximately 1:00pm.*
